自從體外沖擊波碎石術(shù)ESWL（Extracorporeal Shockwave Lithotripsy）上世紀(jì)80年代初開始應(yīng)用于腎結(jié)石的治療以來，體外沖擊波的治療指征已有了很大的擴(kuò)展，體外沖擊波的技術(shù)性能也有了相當(dāng)大的改善。在疼痛康復(fù)領(lǐng)域，上世紀(jì)80年代末發(fā)現(xiàn)體外沖擊波可用于假關(guān)節(jié)和末端病的治療，比如說骨折延遲愈合、上髁炎、肌腱鈣化和足底筋膜炎（跟刺）等。由此體外沖擊波從單一的“體外沖擊波碎石ESWL”演變產(chǎn)生出了新的治療方法“體外沖擊波治療ESWT”（Extracorporeal Shockwave Therapy）。
沖擊波是一種通過物理學(xué)介質(zhì)傳導(dǎo)的機(jī)械性脈沖震波，是壓力急劇變化的產(chǎn)物。在短短的幾納秒內(nèi)產(chǎn)生很高的和瞬間下降的壓力幅度，這是沖擊波獨(dú)有的特性。壓力急劇變化產(chǎn)生的沖擊波具有很強(qiáng)的壓應(yīng)力和張應(yīng)力，所以能夠穿透任何彈性介質(zhì)，如液體和軟組織。早先的體外沖擊波治療仍采用傳統(tǒng)的泌尿科碎石術(shù)ESWL的聚焦型（壓電式、液電式或電磁式）高能量硬式?jīng)_擊波波源，設(shè)備依舊龐大、昂貴和復(fù)雜，對治療的部位需要X線或超聲定位，機(jī)器能量大，需要局部麻醉，治療副作用多且明顯，患者恢復(fù)期較長，臨床也難以得到普及。
